Submitting a Referral
From the top of your dashboard please select the submit referral button:
An additional window will appear as displayed in the attached video clip:
You will only see the forms for your area, these are the services that we are commissioned to look after by your Commissioning Team at NHS England. If the form you require is not there, it is not a discipline we look after.
In some areas you will have access to forms for neighbouring commissioning groups; these should be used in line with the guidance displayed in the yellow box at the top of the form. Community dental service and Oral Surgery providers have strict guidelines related to where the patient and their GP are located
