Referring for Dental Care Professionals in Greater Manchester

Before referring please consider the following:

  1. Whether a referral internally within the practice to a GDP is more appropriate.
  2. if you have discussed this case with a GDP in your practice, it would be beneficial to include the outcome of this discussion in your referral.

Logging in

Please visit https://app.dental-referrals.org/login and enter the username and password fro your practice

The system will present a list of mobile numbers to receive the PIN code to login. If you do not have access to any of these numbers, please ask a colleague to add your number for you (they need to be logged in to do this). You can opt to receive via SMS or phone call and will receive a new PIN each time you log in:

Enter the code and select Log in:

You will be taken to the GDC registration page. If you already logged in and entered your GDC number previously, you can select it from the list. If this is your first time, please enter your GDC number and select lookup my data:

Once you have populated all areas please select "Use this data in referral entry" - Please note that you V- code is your practice ODS Code and can be looked up here. If you are a private practice you may not have a V code and this can be left blank.

You will now be taken to the practice dashboard:

Making a referral

To make a referral select "Submit Referral" to open a list of the available forms for the area your practice is located in, (in this example Greater Manchester):

For this example we are completing the Oral Medicine Form:

The patient's details can be populated in two different ways if you are logging in through the portal as demonstrated here.

  1. Complete the details manually by typing into the boxes
  2. Utilise the NHS demographics option at the top right, this will pull the details from the NHS spine. Complete either side and then select "Lookup Patient Data"

This data can then be transferred to the form using the button at the bottom right:

The details stored on the spine (with the exception of the contact number and email address), are transferred to the form. Now, complete the rest of the form manually:

Depending on the form type and whether it goes via the triage team, you will see different options on the bottom of the form for provider choice (if there is no provider choice this means it will go to nearest as set up by your ICB).

Once the form is complete, you will be presented with the medical history form. If no medical conditions are declared, the form will close to allow you to move on. Otherwise please complete all appropriate sections for the patient:

Following completion of the Medical History Form you will move to the attachments section where radiographs, photographs and other documents can be attached. All radiographs and clinical images should be uploaded in the left hand box:

For images, the system will prompt you to enter the date the image was taken:

This has completed the referral. If you need to send OPG films to us in the post to be scanned please select the grey submit button, if all elements are complete and attached use the red submit button. 

If you need to add anything to the referral i.e. data, images, documents etc, please use save as draft so that you can edit the referral before sending at a later time.
